D-dimer

D-dimer is a clinically proved, high quality reagent for exclusion of deep vein thrombosis (DVT) and pulmonary embolisms (PE). The reagent has a low cut-off with a high sensitivity. The difference between Nordic Biomarker’s three types of D-dimer reagents, is the size of latex particles used to produce them. Apart from that, all three are Latex Immunoassays (LIA) using immunoturbidimetry as detection principle. Also, all three reagents appear white, when looking at them in real life. Their names, Red, Green and Blue D-dimer, are just a mnemonic trick to remember which wavelength of light they are designed for.

PT Quick is a rabbit tissue-based reagent. PT Quick reagent is easy to reconstitute, and calcium is already added to the reagent before lyophilisation. PT Quick is a single vial product calibrated against the most recent WHO thromboplastin standard, RBT/05, which is traceable to the first reference preparation of thromboplastins. The calibration assures low lot-to-lot variation and consequently results in reliable PT determination.

APTT

Activated partial thromboplastin time (APTT) is a commonly used screening test for haemostatic defects in the intrinsic and common pathway of the coagulation cascade, as well as the contact activation system. Thus, FV, FVIII, FIX, FX, FXI, FXII, prekallikrein and high-molecular weight kininogen deficiencies can result in a prolonged APTT. Moreover, APTT is also suitable for monitoring heparin treatment.

The APTT reagent utilises the near colloidal contact activator silica to activate the contact activation system. This ensures sensitivity to mild deficiencies of FVIII, FIX, and FXI. APTT is sensitive to heparin, FVIII, FIX and FXI, while being insensitive to the potential interferent LA. Thus, APTT is suitable for monitoring heparin treatment and for haemophilia screening. APTT reagent contains the contact activator silica and phospholipids to ensure a highly consistent and stable product.

Fib Clauss

Fib Clauss is a functional clotting assay used for quantification of fibrinogen in citrated plasma. Fib Clauss reagent utilise the Clauss technique, where a high concentration of thrombin is added to pre-diluted plasma and the clotting time is measured. The clotting time depends on the level of functional fibrinogen in the sample. Fib Clauss reagent has a large linear range, up to 8 g/L limiting the number of re-analyses.

Thrombin Time

The Thrombin Time (TT) reagent is designed to detect an impaired fibrin polymerisation process and presence of heparin. It can be used as a screening test for functional abnormalities of fibrinogen, and to detect the presence of heparin and/or thrombin inhibitors in citrated plasma. The assay measures the ability of exogenous thrombin to convert fibrinogen to fibrin, followed by fibrin polymerisation and clot detection. TT is a common clotting test used to detect fibrinogen abnormalities, and the presence of heparin and other thrombin inhibitors in human citrated plasma.

PT DOAC

PT DOAC is used for functional determination of the extrinsic pathway of the coagulation cascade in patients treated with direct oral anticoagulants (DOACs).

A simple and rapid assay for functional determination of DOAC effect has been needed since the introduction of DOACs. A major drawback of using conventional coagulation tests for estimation of DOAC effect is the substantial variability in the sensitivity to DOACs amongst different reagents. Nordic Biomarker has taken this disadvantage into account in the development of PT DOAC. This novel prothrombin time (PT) method utilizes a concept already proven for warfarins. As a conventional PT determines the effect of warfarin and not the concentration, PT DOAC determines the effect of DOACs and not the concentration. Furthermore, only one reagent is needed to detect the effect of all the DOACs. The method is patented in Europe (EP31416347) and a world-wide patent (WO2015177293) is pending.

Free Protein S

Free Protein S (FPS) is a latex immunoassay for the quantitative determination of free protein S in citrated plasma. Inherited and acquired protein S deficiency is an important risk factor for deep vein thrombosis (DVT), and pulmonary embolism (PE). Measurements of free protein S can detect more subtypes of protein S deficiency than measurements of total protein S and has therefore become an important tool in clinical thrombosis investigations FPS consists of sub-micron sized polystyrene particles coupled to two different monoclonal antibodies specific for protein S. This system is not sensitive to protein S bound to C4BP, and therefore only reacts with free protein S.

FPS includes two different products designed to match different measuring instruments. For instruments operating in the blue wavelength range (400-600 nm) Nordic Biomarker offers Blue Free Protein S. For instruments operating in the red wavelength range (600-800 nm) Nordic Biomarker offers Red Free Protein S.

FDP

FDP is optimised for instruments in the 500-650 wavelength range and is developed to measure a broader range of fibrin and fibrinogen degradation products (FDP). FDP is a generic name for all fragments formed during plasmin cleavage of fibrin and fibrinogen. The determination of fibrinogen degradation products has become a prevalent aid in the diagnosis of disseminated intravascular coagulation (DIC), pulmonary embolism (PE), deep vein thrombosis (DVT) and acute aortic dissection (AAD). The FDP assay is commonly used in some markets where the laboratories run the FDP assay in parallel with the D-Dimer assay.

Whereas plasmin degradation of fibrin occurs in presence of a fibrin clot, plasmin degradation of fibrinogen occurs during over activation of the coagulation system causing systemic fibrinolysis. During systemic fibrinolysis, seen in patients with DIC, activated plasmin cleaves circulating fibrinogen, rendering elevated levels of fibrinogen degradation products in the blood stream.

vWF Antigen

Green vWF Antigen is a latex immunoassay for quantitative determination of vWF Antigen in citrated human plasma which can be used as an aid in diagnosis of von Willebrand disease (vWD). vWD is the most common hereditary bleeding disorder and is caused by either a quantitative or a qualitative defect of vWF. To discriminate between the different types of vWD, several different lab tests are needed. Green vWF Antigen can determine quantitative defects of vWF and is thereby an important tool to enable correct diagnosis of vWD. Green vWF Antigen consists of two components: Green vWF Antigen Latex Reagent and Green vWF Antigen Reaction Buffer. The latex reagent contains sub-micron sized polystyrene particles to which monoclonal antibodies specific for vWF are coupled. When the latex reagent is exposed to vWF in the presence of the reaction buffer, the particles will agglutinate and the light-scattering of the solution increase. The level of agglutination is proportional to the amount of vWF in the sample.

Antithrombin

Antithrombin (AT) is a liquid, functional, chromogenic assay based on the inhibition of Fxa in the presence of heparin. The chromogenic AT assay measures the AT activity (functional AT) by a two-stage method as follows: An excess of FXa is added to the citrated plasma sample pre-diluted with saline and in the presence of heparin. The residual Fxa cleaves the peptide linked to the chromogenic FXa substrate, which results in the release of free p-nitroaniline (pNA). The amount of released pNA is measured at 405nm and the absorbance is inversely proportional to the AT activity in the patient plasma, expressed as IU/mL or %.

Protein C

Protein C is a chromogenic test for quantitative determination of Protein C activity in citrated human plasma which can be used as an aid in diagnosis of Protein C deficiency. Protein C deficiencies, hereditary and/or acquired, increases the risk for developing thromboembolic disease. There are two different types of Protein C deficiency; type I refers to reduced biosynthesis whereas type II is characterized by a decrease in functional activity. Protein C measures Protein C activity, thus covering both types of Protein C deficiency. Protein C consists of two components: Protein C Activator and Protein C Substrate. When a plasma sample is mixed with the Protein C Activator, all endogenous functional Protein C is activated. The level of activated Protein C in the plasma sample is quantified by adding a substrate that is specifically hydrolyzed by activated Protein C. The pNA released when the substrate is cleaved is proportional to the Protein C level in the sample.
