Galileo Med L

Functional principle of Galileo® Therapy
The principle of Galileo is based on the natural movement of human gait. Galileo's side-alternating motion is similar to a seesaw movement with variable amplitude and frequency, and therefore stimulates a movement pattern similar to human gait. The rapid movement of the training platform causes a tilting movement of the pelvis, just like when walking, but much more frequently. To compensate, the body responds with rhythmic muscle contractions, alternating between the left and right side of the body. From a frequency of about 10 hertz onwards these muscle contractions are not a conscious process but, rather, are a reflex. This stretch reflex activates the muscles in the legs, the stomach and the back right up into the trunk.

The number of stretch reflex contractions per second is determined by the adjustable training frequency. For example, if a training frequency of 25 hertz is selected, 25 cycles of contraction in flexor and extensor muscles occur per second. A training session of 3 minutes at 25 hertz therefore causes the same number of muscle contractions as walking a distance of 4,500 steps.

The vibrations generated by Galileo can be continuously varied in amplitude and frequency independent of body weight.

Frequency and amplitude
The frequency in Hertz (cycles per second) is set on the device and always chosen according to the training objective. Thus, low frequencies are used for mobilisation, intermediate frequencies to train muscle function and high frequencies to increase muscle performance. The amplitude, i.e. the deflection of the training platform upwards or downwards, is determined by the foot position. The farther the foot position from the centre of the plate, the more demanding the training is.

Simulation of human gait
Unlike other training platforms with a vertical (up and down) movement, the tilting movement of Galileo Therapy simulates human gait in a physiological way.

Only by this side-alternating movement during Galileo Therapy is the spine stimulated by a slight lateral tilt of the pelvis. On training platforms with a vertical movement, however, the spine is continually compressed.

The physiological stimulation of the spine by the side-alternating function of Galileo allows the back and abdominal muscles to be targeted. On training platforms with a vertical movement this is possible, but to a very limited extent only.

As humans we attempt to keep the head in a steady and upright position as the balance and visual systems function optimally only when in a state of rest. Due to the design of the machine, no significant vibrations are transferred to the head during Galileo Therapy because the training platform only simulates human gait. The body, therefore, is able to maintain a stable torso and head.
Because Galileo Therapy produces similar stimulus patterns used when walking, in addition to the basic muscle functions itself, the interaction between different muscle groups and their coordination, is developed perfectly resulting in an increase in muscle power. In the elderly especially, muscle power is the relevant factor in preventing falls that can cause hip and femoral fractures. Due to its low burden on the cardiovascular system Galileo Training is ideal for the elderly.

During Galileo Therapy the entire chain of muscles in the leg, and all the way up to the trunk, are targeted. Specific training on individual muscle groups can be achieved by simply altering body posture and tension.

Leonardo Mechanograph®

Leonardo Mechanography
Leonardo mechanography enables objective automatic motion analysis, performance analysis and fitness test. It allows early diagnosis of movement disorders, objective and reproducible assessment of muscular performance and flexibility as well as the documentation of changes.

Product examples

Leonardo Grip Force (GF) Leonardo GRFP Leonardo GRFP LT Leonardo Stair (SC A)

Concept
The main objective of Leonardo Mechanography is to provide efficient quantification of functional human motion based on physical parameters. Leonardo Mechanography analyses functional everyday movement patterns because results are highly reproducible across a large age group from children to the elderly. Leonardo Mechanography offers essential advantages for clinical use, allowing the simple quantification of peak force, peak power and left-right differences of movement patterns. It requires no additional complex measurement devices, no time consuming subject preparation and is easy to use and results are produced within minutes.

Advantages
This concept offers essential advantages for clinical use: for one it allows the objective quantification of actual maximum forces, maximum power and left-right differences during physiological movements. But the method also very easy and very fast to apply in clinical use since no preparation of the subject or complex additional instruments are needed.

Fields of application
Leonardo Mechanography has been used in numerous international research projects. It has been used to characterize different patient cohorts as well as to document and to quantify effects of different interventions. It is for example one of the standard measurement techniques used for bed-rest studies of the European Space Agency (ESA).

Typical fields of application of Leonardo Mechanography:
Geriatrics: Fallrisk, Sarkopenia, Dynapenia, Frailty
Pediatrics: Performance diagnostics, documentation of therapy results
Diagnostics of muscle function
Performance diagnostics
Physiotherapy: Documentation of therapy effects
Athlete Sports: Performance diagnostics

Objektive measurement parameters
Human movement can be quantified objectively based on the physical parameters. Essential for quantification of the power output of the human body are: force, velocity and time. Leonardo Mechanography measures dynamic ground reaction forces and calculates the centre of mass related physical parameters including acceleration, velocity, energy, power, jumping height and stiffness or flexibility.

Measurement method
The variation of the ground reaction force (GRF) over time and space are measured. When two separated platforms for the left and the right leg are used side-differences (asymmetries) during symmetric physiological movements like jumping or chair-rising can be analysed. The acquired spatial resolution of the measured forces allows stabilometric analysis, for example, during various types of balance tests.

Reference Data
Contained reference data

pQCT - the third dimension in bone densitometry.

Peripheral Quantitative Computed Tomography - pQCT

With conventional area-projected bone densitometry techniques (DXA) bone strength cannot be determined adequately because they are lacking information about bone geometry.

With the pQCT technology bone strength with respect to bending, torsion, and compression can be calculated from bone's cross sectional geometry. In addition the results are given as physically correct density units in g/cm³, whereas area projected techniques give only g/cm². Therefore the density values from the pQCT are independent from bone size. This is especially important for measurements on children. With the pQCT technology cortical and trabecular bone can be analysed separately and bone changes can be diagnosed reliably.

Additional morphometric parameter like endosteal and periosteal perimeter and bone cross sectional area are accessible in vivo. pQCT measurements at tibia or radius are performed in a wide range in clinical routine. In paediatrics the advantages of pQCT are especially distinct. It is the only method that allows to differentiate bone growth from other processes. In preclinical research specialised ultra high resolution scanner rare used in many pharmacological studies.

Relation between muscular demands and the resulting bone
But pQCT is even one step further than traditional methods: In addition to bone parameters it also quantifies muscle properties. This allows to quantify the relation between muscle and bone which is essential for the step from Osteoporosis diagnostics to diagnostics of Sarcopenia, Dynapenia and Frailty.

Essential for bone geometry and bone mass is the muscular demand on the bone. Julius Wolf postulated “bone geometry follows function” and Harold Frost specified the more detailed “Mechanostat” theorem. Therefore bone diagnostics must always be accompanied by a sufficient analysis of muscular properties and abilities. pQCT measurements (e.g. at 66% distal tibia) utilize this concept: it allows to quantify the relation between muscular demands and the resulting bone geometry and bone mass on the individual and site-specific level.

This combined analysis is essential to differentiate for example between a primary Osteoporosis and the effects of diuse or lack of mobility. According to Frosts Mechanostat both will result in loss of muscle mass. The difference can therefore not be found in the bone but in its combination with muscle properties and function. Therefore it is essential to quantify the site-specific relation between muscle and bone. In this context the cross-sectional area of the muscle can be used as a surrogate for peak muscular forces, one of the key osteo-anabol parameters. The examples on the right show on the top a healthy muscle-bone relation, in the middle a primary osteoporosis and on the bottom the long-term effects of a spinal cord injury (SCI).

In addition, newest research results show that especially for Sarcopenia, Dynapenia and Frailty the parameter of muscle density (specific property of muscle tissue) in addition to the muscle geometry is essential. The reason for this is currently considered to be mainly the inter- and intra-muscular fatty infiltration. pQCT allowing the combination of geometry and density measurement at an extremely low cost of radiation is therefore the diagnostic method of choice. To add even more details the pQCT based quantification of muscle and bone can be extended by the quantification of muscle function and performance based on the Leonardo Mechanography systems - for example using the complete pQCT Bone & Muscle Bundle.
