BLOG POST: Remote Monitoring Made Easy

Healthcare systems are under strain throughout Europe. We are seeing an explosion in costs, a shortage of skilled workers and physicians while, at the same time, facing an ageing population. However, with intelligent use of new technologies and service models healthcare providers can help improve patient care and support the healthcare system.

Our web-based service platform Bittium MedicalSuite™ is a good example. It facilitates secure information sharing and workflow management between service providers, clinics and specialists. Swiss healthcare provider evismo makes an excellent case of a successful implementation. In cooperation with Bittium, evismo implemented an efficient solution for long-term ECG which makes life easier for both doctors and patients.

The Challenge
According to the World Health Organization (WHO), strokes are among the three most common causes of death worldwide. They often cause long-term disabilities. Up to 80 percent of strokes would be preventable if the main risk factors were monitored at an early stage. In up to 90 percent of the cases, a 24/48 hour ECG Holter monitoring does not, however, provide unambiguous results. This is, of course, very frustrating and even scary. It is especially so for the patients who have to wait for a second appointment. This is why physicians are more in favor of long-term ECGs. The trouble is that in the current healthcare system they often lead to longer waiting times.

An additional challenge comes with the limitation of responsibilities among medical practitioners. In countries like Switzerland, so far only cardiologists can evaluate and agree to a long-term ECG with health insurers. As the first and familiar point of contact for the patient, general practitioners often have no choice but to refer the patient to the cardiac specialist for follow-up examinations, some of which can be complex and lengthy.

Connected ECG Service
Utilizing the MedicalSuite™ technology, Bittium and evismo jointly developed an efficient solution for 1 to 30 day ECG remote monitoring. The resulting evismo-CardioFlex is a wireless, long-term ECG service with maximum diagnostic reliability for general practitioners, cardiologists, clinics, stroke centers, hospitals, and other medical professionals. With CardioFlex the accuracy of the diagnosis is increased by around 10 times compared to the previously standard 24/48-hour Holter.

evismo accesses the ECG and movement data remotely every day and checks if the available information is sufficient for a diagnosis. The vital parameters, which are continuously measured by a lightweight, waterproof ECG sensor Bittium Faros™, are transmitted daily to the evismo platform. The wireless and strongly encrypted transmission of the ECG data is done via a dedicated, highly secure Bittium MedicalSuite Mobile Device with a corresponding app.

The daily data check and ECG analysis is only run until a diagnosis can be made. The ECG sensor is worn for as short a time as possible, but as long as is necessary. This reduces the need for multiple examinations. It makes diagnosis and treatment faster. As a result, patient satisfaction increases and doctors´ and medical staff´s workload gets lighter. It also lowers diagnostic costs. As soon as the sensor can be removed, the patient gets a notification from the special app. The data is analyzed and the referring cardiologist, or a cardiologist from the Swiss evismo service center, then makes a diagnosis. The costs for such ECG examinations are covered in Switzerland by the basic insurance of the Swiss health insurance companies.

In the future, evismo plans to expand the current set-up. The aim is to consolidate the recording and evaluation of other vital parameters, such as blood pressure, oxygen saturation, EEG, etc., on one medical platform. Based on the recorded vital parameters, the company will provide remote monitoring, e.g. for chronically ill patients, or offer medical care at home after hospitalization, whereby patients can be discharged earlier and monitored reliably. Bittium is looking forward to supporting the future endeavors of the dedicated evismo team.

BLOG POST: "We Have a Delivery for You" - ECG On Demand as a Mail-Order Service

Our Medical Technology team puts in long hours of hard work and passion to develop products that help tackle healthcare challenges. The most satisfying reward for us as developers is to see how these products have actually helped to create new services that make the patient’s life easier and safer. One such example is the success story of the ECG On-Demand® mail order service by award-winning UK based Medical Service Provider Technomed Ltd. When the COVID-19 pandemic hit, both primary and secondary care stopped in-house ECG Holter fitting. Technomed demonstrated that high quality cardiac monitoring is now possible without patients having to leave their homes.

The Challenge
The healthcare industry all over Europe is experiencing challenges that include a lack of clinical resources to meet the demand. In the UK, 90 percent of the healthcare services are public sector funded. This means that the National Health Service (NHS) has a fixed amount of funding despite the growing demand, e.g. from an aging population. Even prior to 2020, the growing demands to monitor patients with cardiac symptoms resulted in long waiting lists. At the same time a trend exists to have longer ambulatory ECG monitoring periods because they provide more accurate diagnostic results.This trend feeds the need to have comfortable devices that can be worn for a long periods of time while connecting to a cost-effective, high-quality remote analysis service.

Technomed has been providing ECG On-Demand® services, including immediate 12-lead ECG interpretation and same-day Holter monitor analysis, since 2010. The idea to offer the service as a mail-order was in development early 2020 but the COVID-19 breakout expedited the roll-out. Not only was a mail-order service a very good way to meet the growing demand for ECG services with an efficient and effective means of undertaking diagnostics, it also became a necessity when in-house Holter fitting was no longer possible due to the additional restrictions posed by the pandemic.

The Perfect Fit
The basis for any ECG examination is the recording device itself. Technomed’s Managing Director Mark Hashemi summarizes why they picked Bittium Faros™:  “We needed an ambulatory ECG monitor that was easy enough for a patient to fit themselves whilst providing a high-quality, low artefact ECG recording for our analysts. The Bittium Faros device fulfilled these requirements perfectly. The devices were configured to start recording automatically on skin contact. The waterproof nature of Faros and electrodes meant the monitor was truly “fit and forget” for monitoring for up to 7-days. Our average test success rate of 97 percent for patient self-fit monitoring speaks for itself.”

The waterproof Bittium Faros ECG devices together with Bittium wearable patch electrodes make the patient’s life easier during long-term (3-30 days) ECG recordings. The patch electrode can be easily attached to the chest and it can be used for 3 to 7 days at a time. After this, the measurement can be continued by replacing the patch electrode with a new one. The waterproof devices and patch electrodes allow the monitored person to be active, do sports and shower without having to remove the ECG and stop the recording. The devices are certified as medical CE class IIa and FDA 510(k) and class II medical devices. 

A Genuine Affair of the Heart
The healthcare system is known not to be very quick in adopting new services or processes, like a mail-order service. With the outbreak of COVID-19, however, the demand for a “contactless” method to provide ECG service practically exploded as visits to healthcare facilities were no longer possible for many patients. 

The Technomed team mobilized all its resources. It organized all logistical processes, such as drop-off and pick-up coordination with the patient via a courier service, as well as telephone support to help guide patients seamlessly through the process. It also created a video guide and a step-by-step manual with an accompanying diary App with which symptoms could be recorded. In this crisis it was especially important to make sure patients and practitioners were getting all the support they needed. 

The result is a convenient ‘mail-order Holter kit’. It consists of a pre-programmed Bittium Faros™ Holter device, hypoallergenic long-term electrodes, skin-prep abrasive wipes, disposable razor and comprehensive instructions on how to apply. The kit enables cardiac arrhythmia investigations without having to go to hospital. This helps reduces stress for both the patient and the cardiology department while clinicians quick access to the necessary diagnostic results to initiate treatment.

High Demand and International Interest
The feedback received from physicians praises the innovation. The service has dramatically reduced wait-times while allowing essential cardiology diagnostics to continue without the need for a face-to-face appointment. The quality of the cardio scans and the resulting reports exceeded the expectations of most clinics. Patients report that the service is professional, efficient, and application easy to undertake.

Predominantly, the Technomed mail-order service has been adopted by NHS hospital trusts. Initially it was a means to keep services running in the early days of COVID-19. Subsequently it has helped clear backlog in diagnostics. Today Technomed works with leading NHS and private hospitals in the UK, including the most prominent London-based teaching hospital. International co-operations utilizing Bittium Faros include research projects with institutions such as the German Institute of Cardiovascular Medicine, the Herzinstitut Berlin.

Bittium Biosignals Ltd, a Subsidiary of Bittium Corporation, and Preventice Technologies, Inc. Have Signed an Extension to Their Supply Agreement of ECG Monitoring Devices

Bittium Biosignals Ltd, a subsidiary of Bittium Corporation, and Preventice Technologies, Inc., a subsidiary of Preventice Solutions, Inc., and a leader in digital healthcare solutions and remote cardiac monitoring services powered by deep learning and artificial intelligence, have signed a supply agreement under which Bittium will supply Preventice with BodyGuardian® MINI cardiac arrhythmia monitors. This agreement is an extension to the supply agreement between Bittium and Preventice, signed in February 2018, under which Bittium has supplied customized wireless ECG monitoring devices during the last three years. Bittium issued a stock exchange release in reference to the original agreement on February 13, 2018.

If materialized in full, the total value of the agreement signed today is approximately USD 22.9 million, (approximately EUR 19.4 million based on an exchange rate of September 9, 2020), with revenues recognized gradually during the years 2021, 2022, 2023 and 2024. This agreement does not change Bittium's 2020 financial outlook published in the Half Year Financial Report January-June 2020 on August 6, 2020.

BLOG POST: Bittium BrainStatus™ Helps Solving Challenges in Neurological Emergency, Acute and Intensive Care Medicine

Neurological emergency medicine, alongside surgery and internal medicine, has become the third most important area in hospital emergency admission [i]. According to the World Health Organization, 15 million people suffer a stroke worldwide each year. Other neurological emergencies that paramedics and medical personnel in emergency rooms and clinics are regularly confronted with include epileptic seizures, cerebral haemorrhages, or traumatic brain injuries. Electroencephalography (EEG) provides the basis for diagnostic decision-making for the treatment. However, speedy EEG examinations have hitherto been associated with a whole series of challenges – especially in emergency and acute medicine. Bittium has tackled these challenges with the new BrainStatus™ EEG solution, which employs a new generation of electrodes, paired with analysis functions based on artificial intelligence (AI).


New EEG Electrode Significantly Speeds Up Starting the Measurements
The biggest challenge, in carrying out an EEG quickly, is the application of the electrodes. For conventional medical EEG devices 20 or more electrodes have to be applied over the entire head of the patient, with special care to ensure skin contact in the areas covered by hair. A practiced EEG-MTA usually needs about 20 minutes to attach the electrodes – or still 10 minutes for an EEG cap. This causes two major problems: the time factor, which plays a decisive role in emergency medicine, and the need to have access to specially trained staff at all times. Ambulances cannot be equipped with specialists for all possible medical incidents, and EEG-MTAs and neurologists cannot be even on-site in emergency rooms 24 hours a day either.

Bittium developed a new electrode system with technology from the field of wearables. As part of the medtech solution BrainStatus™, a band with a limited number of electrodes, which are applied to the hairless area in front the head, can deliver precise EEG signals. The electrodes do not have to be attached individually, and the exact positioning is already specified.

The advantages are obvious: the electrodes can be attached much faster, and it is so uncomplicated that paramedics and emergency personnel can derive an EEG without special training.

EEG Without Mobility Restrains
Another disadvantage of previous EEG electrodes was their complex wiring, which hindered treatment, especially in emergency medicine. In an emergency room, several doctors and medical personnel often have to treat the patient at the same time. An EEG system with conventional electrodes, which are connected to a monitor via cables, is very cumbersome. The systems were even less suitable for ambulances and patient transport.

Bittium used technology known from wearables to design the mobile EEG BrainStatus™. The electrode band is connected directly to a very compact transmitter that displays the signals wirelessly on a monitor, without impairing the mobility of the patient. This transmission ensures high signal quality and also enables long-term EEG measurements.
Simplified Data Analysis with AI Technology
Traditionally EEG recording is monitored visually and up to now, the data could only be evaluated by trained specialists and was very time intensive.

The specialists don't usually have the time to sit next to a patient for a long period of time to view the recordings in real-time. In the retrospective evaluation, large amounts of records had to be checked and evaluated in order to identify deviations and new incidents. Since the patients are mostly sedated, this also causes the risk that no one would notice another attack.

This topic is not only relevant for emergency and intensive care medicine, but also for long-term monitoring of patients with migraine headaches or sleep disorders.

BrainStatus™ meets this challenge by applying intelligent algorithms to analyze the EEG recordings. While this cannot replace neurologists or trained MTAs, it is a great relief and time-saving measurement the medical staff benefits from enormously when it comes to patient care.

The AI functions evaluate the EEG recordings to create an overview of the recorded data and evaluate trends. Based on this simplified data display on an easy to use tablet, the staff can immediately see if all values are in the “green zone" or if there has been a new incident recently. If the values are outside the norm, the responsible doctor or a specialist can be consulted immediately to take a closer look at the corresponding more detailed recordings and measurements.

The system's software covers all of the standard functions of modern EEG systems, and is also easy and intuitive to use for medical and care personnel who are not familiar with EEG software.

Remote Monitoring for More Flexibility and Faster Specialist Support
In addition to recording the data in the device's memory, the patient's EEG data can be transferred to a data storage via the clinic network, so that a neurologist can immediately view the recordings and analytics and give instructions from any location in real time. If there is no specialist on-site, the system also enable reviewing by specialist clinics or experts at other locations. This not only saves time in everyday clinical practice – it can also increase the chances of survival in acute emergencies.

Since the device is not only easier to use, but also compact and suitable for mobile use, even rescue workers can apply the EEG electrodes in the field and transfer the data to specialists, instead of just taking standard measures before transport. Remote monitoring for long-term recordings can also be used on an outpatient basis in the future. This would not only make life easier for patients and clinic staff, but also enable cost savings. After applying the electrodes and the compact transmitter, the patients can leave the clinic and, for example, perform a 24-hour EEG recording at home.

BLOG POST: COVID-19: Bittium Faros Echoes Its Name as a Cardiac Lighthouse Through Mail ECG Service

The name Faros is an inspiration from the Lighthouse (Pharos) of Alexandria built by the Ptolemaic Kingdom. One of the Seven Wonders of the Ancient World, for many centuries it was one of the tallest man-made structures in the world. The name "Faros" was coined just as the original Pharos to denote a lighthouse that gives direction to ships. Bittium Faros™ sailing themed nomenclature connotes a similar mindset; albeit applicable in the field of cardiac arrhythmia diagnostics. A reason why the Bittium Cardiac solutions adopt names like Faros (ECG device), Cardiac Navigator™ (Holter software) and Cardiac Explorer™ (Event monitoring software). The connecting story is that the Bittium Faros series serves as a "cardiac lighthouse" directing cardiologists to the right diagnosis. This has been a driving force among users of the Bittium Faros series of cardiac devices and solutions.

Customer Ingenuity Powered by Bittium Faros
The on-set of COVID-19 brought untold challenges. A need to practice social distancing and remote working required that healthcare professionals needed to rethink how they executed their tasks. For those in cardiac diagnostics sector where applying ECG devices in most cases required personal contact, a lot of these planned diagnoses had to be postponed. Our partner in the UK, Technomed Limited (operating as ECG On-demand) for example, explains that “Pre-COVID-19, waiting times for Holter monitoring of several months were not uncommon and in some UK regions, they extended to over a year. Post-COVID-19, the situation has deteriorated dramatically with the majority of cardiology hospital outpatient appointments cancelled or postponed due to the transfer of cardiac physiology staff to intensive care".

Thanks to the ingenuity of our customers and the unique features and user-friendliness of Bittium Faros, cardiac service providers have started mailing deliveries for Holter ECG services. This new COVID-19 induced solution means that patients requiring ECG diagnosis are allowed to do so without compromising the social distancing rules and equally minimizing exposure to the virus. Sanitised Bittium Faros devices are sent via a mail-order system with clear and simple instructions; in some cases, support can be provided via telephone or video calls. At the end of the recording, the device is collected for data retrieval and analysis. Subsequently, the final report is delivered electronically. Apart from those in the clinical sector, some cardiac research institutions are also using a similar approach to continue their cardiac research efforts during the pandemic period.

Reaching Out To Global Markets
There are already examples of this use case in Europe, Africa and the US; expectedly, this service should extend to other parts of the globe as we continue to battle and win the collective fight against COVID-19. Technomed believes that now is not the time to halt Holters and which is why they mobilized their service offering to include Holter monitors by courier to keep patient visits to the hospital or surgery to a minimum. Our partner in South Africa, Vertice Diagnostics, are of a similar opinion and have also commenced "a Holter by mail" service. Bittium BrainStatus™ product family has received medical device approval in Europe (directive 93/42/EEC). Bittium BrainStatus™ product family is a compact wireless EEG measurement system for the brain's real-time electrical activity monitoring. It is designed for emergency, acute and intensive care use. Its ease of use enables high-quality EEG measurements by both paramedics and clinicians quickly and easily.

Bittium BrainStatus™ product family consists of a wireless compact EEG amplifier, EEG measurement software, and a disposable electrode headband that is quick to wear below the hairline. Bittium BrainStatus can be used for real-time remote EEG monitoring for patients with altered levels of consciousness e.g. epileptic seizures or detecting effects of brain hypoxia.  The system enables quick measurements also in field conditions. Remote diagnosis can be done regardless of the patient's or clinician's location, which makes the treatment process faster for the patient.

Bittium BrainStatus™ device is a compact, wireless and rechargeable EEG amplifier designed for use with Bittium BrainStatus electrode. The amplifier is lightweight and easy to use with high signal quality, enabling long term EEG measurements.

Bittium BrainStatus™ electrode is a novel EEG electrode, made to answer the diagnostics problems in emergency dispatch and other applications which require fast and easy EEG readouts. Bittium BrainStatus electrode is disposable, and unlike traditional headbands, it is placed on the hairless area on the patient's head.
