LBR Med

All robot capabilities that are particularly required in medical technology are bundled by the LBR Med. KUKA supplies the LBR Med as a robotic component for integration into a medical product. This integration is surprisingly easy, as KUKA provides you with a CB Report in accordance with IEC 60601-1 and IEC 62304 for the LBR Med.
In 2018 the KUKA LBR Med was honored with the renowned IERA Award. The KUKA LBR Med convinced the jury thanks to its wide range of potential applications, the safety technologies required for integration into medical products and the great importance of medical therapy, rehabilitation and nursing care in an aging society.

LBR Med - Features

As a precise and reliable assistant in the medical environment, the KUKA lightweight robot LBR Med supports doctors and therapists. At the same time, it is the first fully-fledged robot to be certified according to the CB Scheme procedure as a robotic component of a medical device. The LBR Med thus enjoys a unique selling proposition in medical robotics. At Medica 2019, KUKA will be presenting the versatile application areas of the sensitive robot in five sample applications. For this purpose, a demo cell was designed in which these can be displayed with just one LBR Med. The operator can select which demo to show on a screen. A tool change stand provides the various medical devices for each demo.

Ultrasound demo:
An ultrasonic transducer attached to the flange of the LBR Med is guided over the body to be examined. The path required for the examination is set manually by the operator by moving the robot with the ultrasonic probe along the points to be examined. The robot records the path internally and can then repeat the learned path independently and with constant pressure.

Trocar application:
The trocar application illustrates how the robot can fix a specific entry point manually defined by the operator for minimally invasive surgery. The robot memorizes the entry point and allows the operator to manually move the robot around this point without exerting any force on the entry point, the so-called trocar point. This helps to minimize the patient injuries during surgery.

Spinal column application:
The spinal column application demonstrates that the robot can be used like an intelligent, precisely positioned holding arm. It can be controlled, moved and fixed with any medical device, e.g. a guide for a drill or catheter. In addition, the operator can manually guide the robot along the spine. Any desired point can be fixed and memorized. The robot could also be connected to a therapy planning system and commanded by it. Before the surgery, the target positions are determined using the patient images, e.g. from MRT or X-ray examinations and those positions are communicated to the LBR Med.

Elbow application:
The robot is kinematically overdesigned with its seven serially arranged axes. The elbow application demonstrates the possible zero space movement. The robot keeps its tool center point constant on the path or at a fixed position in space, while the elbow of the robot can be moved. The sensitivity of the robot is also demonstrated, because the user can manipulate the elbow of the robot manually, while the robot itself carries out its task unperturbed.

2D path application:
In order to demonstrate that the LBR Med can also perform pure robot tasks, a 2D path application is presented. It precisely follows rigid movements. A path printed on the table of the demo application is traversed continuously and alternately – once going directly from point to point and once using a calculated rounded path between points. This makes it clear to the viewer that a therapy planning system can also calculate complex therapy paths and send them to the robot for execution.

If the demo is changed, a tool change may be necessary. To do this, the robot moves to the tool change stand, where the user can remove the current tool from the flange and replace it with the new one. An ingenious currentless mechanism is used, which firmly holds the tool magnetically to the flange and releases it very easily by manually moving a lever. Shifting the lever causes the magnetic field either to flow through a metal plate in the tool, fixing the tool to the flange, or to short-circuit the magnetic field inside the flange and release the tool. This technique has already impressed customers because it allows the field strength of the integrated permanent magnet to be used to set a desired breakaway or shear force, making it easier to solve the MRK questions of risk analysis in one application or another.

Accuray: Robot-guided radiation therapy
Since 2000, KUKA has been working in partnership with Accuray. From this partnership, the first commercially available robot-guided radiation surgery system arose. Since then, the CyberKnife system has been brought into use for high-precision tumor treatment at leading radiation therapy centers throughout the world. The surgeon uses a bundled, high-energy X-ray beam instead of a scalpel. As the patient lies on the operating table this beam is guided around him by a robot arm so that the dose is concentrated in the tumor. An imaging system records the position of the tumor and signals any movement to the robot, which then compensates for these motions. Thus, the CyberKnife is able to hit tumors with pinpoint accuracy leaving the surrounding healthy tissue undamaged, regardless of where in the body the tumor is located.

KUKA Innovation Award 2020 – Team CoNeeBot (Hamburg University of Technology, Germany)

Smart needle with collaborative robot control

Needles are a key tool for minimally invasive treatments such as biopsies. So far, robotics and image guidance have primarily been used to align the needle with respect to a target structure, while advancing the needle was left to the physicians. However, often the insertion is also difficult, as soft tissue deformation and needle bending complicate precise placement and may result in missing the target. Team CoNeeBot’s goal is developing a collaborative robotic system which provides assistance during the complete needle placement procedure. A smart needle is used to sense the tissue in front of its tip. AI methods convert this into information, e.g., on mechanic properties or distance to target structures. Using the collaborative robot this information is provided  to the physician in a natural way through haptic feedback, virtually putting the physician’s finger directly at the needle tip. The CoNeeBot system will improve safety and accuracy of needle insertion procedures with an easy integration into the clinical workflow.

KUKA Innovation Award 2020 – Team HIFUSK (Scuola Superiore Sant’Anna, Italy)

High Intensity Focused Ultrasound Surgery based on KUKA robot

HIFUSK can address unmet clinical needs in the treatment of pathological tissues, like cancerous tissues. HIFUSK is the marriage of 2 technologies: robotics and focused ultrasound. Such a combination results in a surgical treatment which is precise thanks to robotics and non-invasive (no incisions, no anesthesia, no ionizing energy) thanks to focused ultrasound. Shortly, a focused ultrasound transducer is mounted on the KUKA robot together with a confocal ultrasound probe, and it is placed in contact with the patient’s skin. The focused ultrasound transducer provides the energy to treat tissue deep in the body without damaging the surrounding tissues, while the ultrasound probe works as the eye to guide the therapy and monitor the effect. Robotic control and machine learning algorithms ensure safety even in case of target motions during the therapy. We validated our platform in a dry lab/ex-vivo scenario and we received the approval for in-vivo tests of HIFU robotic treatments.

KUKA Innovation Award 2020 – Team RAOCT (Duke University, USA)

Robotically aligned optical coherence tomography

Optical coherence tomography (OCT) has revolutionized eye examination by providing quantitative imaging of key ocular structures, especially the retina and cornea. Although this imaging technique is considered standard for management of common eye diseases, such as glaucoma or diabetic retinopathy, clinical OCT systems require extensive training and expertise to operate, which limits their use to eye specialty clinics. In our demo, we show a contactless robotic OCT system that greatly simplifies the process of OCT imaging and can even complete imaging sessions autonomously. Our system features face-tracking cameras to guide the robot to the subject’s eye as well as high-resolution pupil tracking cameras integrated into the imager for optical motion compensation purposes. In autonomous mode, our system issues voice prompts to guide subjects through the imaging process without any operator intervention. This approach promises to fundamentally change the practice of eye examination.

Semi-autonomous hair removal robot assistant

During laser hair removal, a highly concentrated light enters the hair follicle. There, the laser light is then absorbed by the pigment at the hair root and generates heat. This damages the hair follicle and thus inhibits future hair growth. Team SAHARRA is developing a robot application to improve the precision and speed of the treatment. To achieve this, a navigation system determines the exact areas to be treated in conjunction with an LBR Med. In this way unnecessary double treatments can be avoided.

KUKA Innovation Award 2020 – Team SpheriObot (Shanghai Jiaotong University Affiliated Sixth People's Hospital, Shanghai Jiaotong University, Shanghai Electric Group, China)

Collaborative robot for spherical osteotomy

There are many patients with hip problems, including young people. The traditional treatment method for hip dysplasia – an insufficiently formed acetabulum – is periacetabular osteotomy (PAO). This unwieldy name refers to a procedure in which the hip socket is cut out and repositioned through several incisions. This can lead to damage of nerves or blood vessels. The team from China is developing a robot system that can reposition the hip socket in a targeted manner with fewer incisions thanks to a special saw. This can increase the precision of the operating method and thus lower the risk of surgical trauma.
