PERIPROBE® - PERINEAL PROBES FOR THE TREATMENT OF INCONTINENCE BY ELECTROSTIMULATION & BIOFEEDBACK

Urinary incontinence is a problem affecting many people, predominantly female for anatomical reasons. Perineal electro-stimulation and biofeedback techniques allow to solve a very high percentage of cases, alternatively or in combination with surgical techniques. Different types of urinary incontinence have been recognised and codified: URGE incontinence, STRESS incontinence, MIXED incontinence and OVERFLOW incontinence.
Perineal electrostimulation
The reeducation of urinary incontinence exploit the perineal electro-stimulation to obtain inhibitory action on involuntary contractions of the bladder muscle (detrusor), in case of URGE incontinence (detrusor instability). In the cases of stress incontinence, the perineal electrostimulation allows to develop muscle tone of the pelvic floor and improve the control of continence, during abrupt variation in abdominal pressure (effort, coughing, sneezing ...). Alternatively to the vaginal access, the pelvic floor muscles are also accessible via the anus. For this reason, perineal stimulation can be performed with vaginal probes, in adult female patients, or anal probes in the male patients, or when the vaginal access is unsuitable or not recommended.
Perineal biofeedback
Another therapeutic possibility for incontinence is represented by electromyographic (EMG) or manometric biofeedback techniques. The first one consists of detecting the muscular action potential generated by the pelvic floor muscles, the second one consists of detecting the intra-vaginal pressure during the contraction of the involved muscles. The therapy using biofeedback promotes awareness and voluntary control, by the patient, of the pelvic floor, in circumstances that could cause urinary incontinence.
Perineal electro-stimulation and perineal biofeedback are not alternative but complementary therapeutic techniques. Both perineal electrostimulation and perineal biofeedback involve the use of special vaginal or anal probes.

PERISPHERA® - PERINEAL PROBES FOR THE TREATMENT OF INCONTINENCE BY ELECTROSTIMULATION & EMG BIOFEEDBACK

The most effective treatments require the most appropriate tools.
Several hundred million people worldwide suffer from urinary incontinence and most are women.
The incontinence can have different causes and forms. The most common forms are: stress incontinence, urge incontinence and mixed incontinence.
The good news is that, with incontinence, effective treatments are available.
The sooner incontinence is diagnosed and treated, the better the chances of success.
Perineal electrostimulation and biofeedback are the most popular methods in the recovery of continence, particularly in female patients.
These methods involve the use of intra-vaginal probes, characterized by at least one pair of electrically conductive electrodes.
Despite perineal electrical stimulation and EMG biofeedback, being well known and commonly applied to treat incontinence, some perineal probes that are available on the market, could be unsuitable for the most effective treatment protocols.
Frequently, the therapeutic successes are limited by a bad localization in the treatment or by the discomfort for the patient.
Very frequently, the pelvic floor of a woman shows pronounced asymmetries of muscular tone; They may be caused by an episiotomy or asymmetrical lacerations during childbirth. Advanced vaginal probes allow to assess or selectively treat the perineal muscles right or left with different levels of stimulation.
Moreover, since the nickel allergy is increasing worldwide, it is increasingly important to avoid contact with metals containing nickel (for example with stainless steel) and use anallergic materials for the contact with vaginal and anal mucous.
These are the principles that inspired the development and production of a new line of vaginal and anal perineal probes.
The particular forms of PERISPHERA probes make it easier to get a strong, comfortable contraction, or a precise ElectroMyoGraphic (EMG) recording, during a voluntary contraction. The hemispherical and protruding electrodes offer the maximum contact surface and a better localization of the treatment.

The evoStim® P & E units are unique medical devices, offering professional features at a typical friendliness of equipments for home-care. They are suitable to perform passive treatments (by electrical stimulation), active recover (by biofeedback) and muscular re-learning (by triggered stimulation). The only difference between the models P and E is the biofeedback signal input: the P model is for pressure biofeedback and pressure-triggered stimulation (more suitable for perineal rehabilitation) while the E model is for EMG biofeedback and EMG-triggered stimulation.
Performing biofeedback with evoStim® P & E is easy, thanks to the pre-set parameters and the self- calibration capability. The visual feedback may be switched between bar-graph and sphincter-graph. With evoStim® E, electrical stimulation, biofeedback and EMG-triggered stimulation may be applied on external skin areas (by surface electrodes) or in intra-cavitary way (by vaginal or anal probes). The package includes only 4 transcutaneous electrodes, the probe, possibly necessary for perineal treatments, can be chosen in the range of accessories and ordered separately. The electrostimulation with evoStim® wholly exploit the Intellistim® function: after selecting the therapeutic purpose with one of the quick-selection touch-buttons (URGE-STRE-MIX-PAIN), if you change the frequency, all other time-related parameters will be automatically adjusted. This is equivalent to having an infinite number of electrical stimulation programs stored.
The evoked stimulation program (EVO), with auto-increasing thresholds, allows performing the EMG-triggered stimulation also called “muscular cognitive relearning”, which is an affective training based both on the voluntary residual contractions of the patient and the benefits of electrical stimulation.
