New device helps paraplegic women catheterize independently

More than three million women worldwide live with paraplegia, often as a result of accidents or neurological diseases such as multiple sclerosis. Many rely on catheters to empty their bladder, as paralysis prevents natural urination. The process, repeated up to eight times per day, is vital for survival, yet for women, it often requires external assistance due to anatomical differences.

Permanent catheters pose a high infection risk, making intermittent catheterization the safer option. However, since the female urethra is not externally visible, women often depend on nursing staff for help, an intimate situation that can feel uncomfortable and increase the risk of urinary tract infections (UTIs) and bladder cancer.

To address this, Dr. Urs Schneider, physician and scientific director for Health and Bioproduction Technology at Fraunhofer IPA, and his team launched the 2LIP project. Their goal: to create a self-catheterization device that restores women’s independence and dignity.

“The situation on the toilet is often very stressful for those affected, causing them embarrassment,” says Dr. Schneider. “Our device is designed to help women with paraplegia regain their sense of privacy.”

The new system includes an ergonomically designed seat unit that can be used directly from a wheelchair. Two flexible shells position the legs gently apart, only 20 degrees instead of the previous 110 degrees.
A lever mechanism allows users to open the labia with minimal effort, while an integrated mirror and light provide clear visibility of the genital area. This design enables hygienic and independent catheterization, eliminating the need for external help.

The technology not only enhances autonomy and privacy for patients but also reduces the workload for nurses. It saves time, relieves psychological strain, and improves overall care efficiency.
Initial prototype testing has received positive feedback from patients, caregivers and insurers. The research team is currently optimizing the design with multiple geometric variations to accommodate different anatomies.

Encouraged by these results, the team plans to establish a start-up and conduct a comprehensive clinical study. Within the next few years, the device could become available on the market — marking an important step toward greater equality and independence in women’s healthcare.
